Case Management
Goals of Case Management:
Is to assist client(s) in learning how to live full and productive lives without drugs or alcohol. Teaching the client how to practice recovery skills, maintain abstinence from drugs and/or alcohol, setting goals for employment & education, and building healthy relationships for a new life.
Who can benefit?
Case Management is designed to support individuals completing treatment programs as well as families needing professional support, whether their loved one enters a treatment program or is still deciding whether they will accept treatment.
Case Management – May include, but is not limited to:
- Virtual or in-person meetings
- Ongoing assessment and treatment planning
- Referrals to additional professionals
- Collaborating with additional professionals
- Virtual conference calls
- Telephone consultation
- 24/7 Text message support
- Recovery coaching & monitoring
- Sober link testing
- Signed releases (HIPAA Laws)
- 12 step meetings
- Recovery support groups
- Provide drug and alcohol screening/random testing
